A Place to Report Accidents - Approved

As of right now, there is no 100% respectful and official way to approach an accident that occurs in the running of site systems, as rare as they are... Especially if one doesn't know which moderator accidentally made the mistake.
It may be super embarrassing to notice one made a mistake! Everyone knows that awkward feeling when they notice they made a mistake. In addition, folks can feel like they have been called out if contacted in public. Everyone is human, so mistakes happen!
IE: Contacting the NPC account via message is sometimes not noticed or may miss the individual that needs to be contacted, especially if you don't know who to contact properly. Mods only really get on the NPC account, from the looks of it, to do shop replies, so it may be a while if there is a lull in shops needing replying that stuff may not bee seen unfortunately. Also, not everyone is on the site discord, so it isn't always possible (and may not even be appropriate or taken well) to contact someone there.

So, my suggestion is: Create a Request area thread or something similar where a person can report an accident respectfully and without singling out anyone that may have had an accident. Accidents just happen sometimes, and no one needs to be embarrassed! :3
-Accidents would NOT be something like 'please roll my thread' (that would be basically harassment). Especially since mods don't always have the time to do stuff like this.
-I'm talking about things like being delivered a quest prize, but not getting the QP added to the Bank (it happens, sometimes). Or perhaps not being sent an item by the NPC account after having bought them (happens usually when folks have a large list since it is easy to overlook things).

Another option would be informing individuals who to get a-hold of when there is an accident, perhaps in a tutorial or somewhere else easily findable. c:

If you have other ideas that could work better, that is fine. Whatever works to help keep the situation simple on all sides, and a place where people can easily find out who to contact or what to do in order to keep from hurting each other.
